Data point...
I had sort of noticed this before, out of the corner of my eye, because sometimes, when I'm out and about and I click on an attachment that is shown at the bottom of an email on my iPhone, I am informed that there will be a slight pause while said attachment is downloading.
But not always. Some attachments seem to be already on my iPhone, waiting for me.
I just spent a little time online to see if there was either a way to limit the size of automatically downloaded attachments (i.e., don't download if over, say, 100 KB in size), or if that aspect of configuration is out of my hands.
So far, it would appear that attachments greater than 1 MB have to be specifically "asked for."
Why do I ask, you ask?
Because Verizon's "international data" plan provides for only a meager 100 MB of data before charging me another $25 for the right to receive a similarly sized slug of data.
Yeow!
