It's having both sides use revision mode to make iterative revisions, with no apparent coordination between iterations, which means - among other things - having the translator do the rework in revision mode.
The possibilities are truly mind-boggling.
It's gotten so bad that I've created macros to insert comments that read, e.g., "This text appears in the Russian source, but not as a revision." It's very useful when flagging "unrevised" text in the Russian source that doesn't exist in the English version of the document.
